I am a new T-mobile customer and I brought my device over from my old carrier. Where do I submit my documents showing the balance of my devices to receive the virtual cards?

Here’s a link for carrier freedom. Remember you still need to pay off any ETF’s to your old carrier.
Tmobile will send you a prepaid card toward the expenses.
