I paid the deductible and they said I would get an email with an address to take my phone to get it repaired and I haven't received the email. My phone is still broken, I can't see a whole corner of my screen. I am not happy with the service I have been getting the last 6 months. 

Have you checked your spam folder?  If it isn't in there.  Reach out to T-Mobile by sending them a DM on Facebook or Twitter and they should be able to send you what you need through there.
