stating T-Mobile is requesting my feedback,
is this a T-Mobile sponsored survey or scam?

What number did it come from? Always assume it’s a scam until proven or you know it isn’t.
It was an email and not in spam?

I received an email with survey stating that it is t Mobil survey. I figured I would answer the questions now it is going to charge my card over 56.00 per month which I never authorized. They offered a free gift with just the shipping cost if you take the survey. After I did that now they are going to charge my card that much a month for a year. What do I do

That sounds like you were scammed! T-Mobile doesn't send out e-mail surveys.

Could be a scam. The survey I just got from T-Mobile was from https://customerexperience.t-mobile.com
