T-Mobile customer support unable to unlock my phone since 12/1/17

    Despite several customer service requests and escalations since 12/1/17, T-Mobile tech support has not unlocked my LG-B450 flip phone, that qualifies for unlock. Although tech support provided unlock code via email, this code did not unlock phone with T-Mobile sim card despite several attempts. Previously, I escalated issue several times via tech support supervisors to no avail. Please help me unlock phone.
