I have a Magenta® MAX 55+ service for 3 people, and wonder what Spotify service is included? Is it one individual account, 3 individual accounts or one Family account?

I don’t believe any plans had Spotify subscription. They had plans where data didn’t count when using the services.

Spotify was showing in that link but the streaming was included not the plan.
The issue remains a bit foggy, and it is clear that the T-Mob folks you talk to have limited understanding of the offerings they are selling...

Every phone is eligible for unlimited streaming for music as long as it's listed on the support page.
