My bill is showing I have 6 lines. I only have 2 and was sent a sim card to put in another phone we want to bring over. I have not inserted the sim card yet as I was supposed to get a phone call to set that one up. So I should only have 3 lines, 2 of which have only been used.

by any chance did you receive any watches with your service? if so they will show as lines on you account. if not try calling one of the lines listed and see what happens. if someone answers, contact T-Mobile and let them know there are lines on your account that are not yours and have them take action to find out how this happened. and as a temp. solution you can suspend the lines that are not supposed to be there.
