My wife and I both have T mobile plans but we are on separate accounts. I want to buy her a apple watch with mobile service and pay for the watch and the service on my account. Is this possible?

It is. The EIP, if that is the route you are going, just binds the purchase to your account. The watch can be activated for your wife without any issues but she will have to activate on her account. When you purchase it though, make sure you tell the sales rep it will be a gift and the device will be used on another T-Mobile account.
