Rebate Website not Working

Hello, 

We recently switched from Verizon to T-Mobile. When we switched, we were told that we would get a $200 rebate for each line ported over. When I go to promotions.t-mobile.com I am able to enter our phone number (the one the account is set up under) and sometimes I can log in. However, once I either log-in or even just put the phone number in, I get a message that says “whoops, something went wrong”. We have tried for a couple days at separate times but we can’t get passed the log in phase. Is there something I am doing wrong on my end? Is there a phone number I can call to get this done? This is extremely frustrating because we would get about $1000 in rebates and we have a time limit to submit the claim.

This is what I see when I get passed the phone number/log-in info

 

When I try to use promotions.t-mobile.com it puts me in an endless refresh loop after I enter my T-Mobile phone number.
